Membership The following describes the criteria and fees for membership in the C.S.A.O. REGULAR MEMBERS: All those people who are occupationally or professionally involved in the instrument/device reprocessing in a Health Care setting. "Regular Member in good standing" is a member who has paid Association Membership fees for the current year. ASSOCIATE MEMBERS:(Excluding voting and office holding privileges) Those people who are not actively involved in the instrument/device reprocessing in a Health Care setting. E.g. - Directors of Nursing, Purchasing Agents, Infection Control Officers, Manufacturer's Representatives and those former members who have retired. HONORARY MEMBERS: Honorary Lifetime Membership may be bestowed upon an individual who has made an outstanding contribution to the Association as a whole over a period of time. The Executive Committee shall present a recommendation at the annual General Meeting for approval by members present. Honorary Lifetime Membership status shall be "Associate Member" with no fees required. Honorary members are : Tom Preston, Phyllis Norton, Marjorie Brown, Isobel Campbell (deceased) Sister Mary Rose, Mildred Asling, Betty Jane Fielding/Tayler, Pauline Fallis, Larry Paquet, Nancy Hanwell, Inez McGee, Marilyn Toppozini, Muriel Shaw, Margaret Valcour, Butch Benoit and Derek Canfield, Colleen Landers, Bill McInerney, Bob Bothwell, and Dennis Thompson. FEES: The Annual fees shall be as determined by the membership at an Annual General Meeting. Membership fees shall be paid for the entire "Membership Year" of the Association. From each regular membership paid to the Central Service Association of Ontario, 20% will be reimbursed at the end of the fiscal year to each Chapter by the treasurer following receipts of a yearly financial report. Membership fees shall be collected at the Provincial level. Membership fee accompanied by a completed application form shall be sent to the C.S.A.O. office. The membership/fiscal year shall be from June 1st to May 31st of the following year. The financial year is from June 1st to May 31st to enable a financial review. Registration for membership closes each year May 31st for submitting payment. Any registration received after May 31st will be put towards the next year's membership fee. A card and receipt will be mailed to each applicant once your form and payment are received. If any information is inaccurate when the card and receipt are received please contact the membership executive person to make the correction immediately. MEMBERSHIP FORMS: 2007-2008 Membership: This membership starts as of October 1, 2007 and will expire May 31, 2008. This was one of the constitutional changes that passed at the Annual Meeting in September 2007. 2008-2009 Membership: This membership begins June 1, 2008 and expires May 31, 2009 NOTE: You must be a member of the 2007-2008 membership year to attend the Annual Meeting at the C.S.A.O. Conference in September of 2008. |
